  • Health Forms

All required health forms should be completed on the Magnus Health website. Medical evaluations are valid for 13 months and are required to participate in SLS athletics. For questions regarding Magnus Health and health form requirements, please get in touch with Nicole Guido or Irene Maier.
  • Doctor's Visit

SLS requires a doctor's note whenever a student-athlete is evaluated by a medical professional and cleared to participate. To send a note by fax, the number is 203-972-5304. By email, guidon@stlukesct.org.

  • What is ImPACT Baseline Testing?

SLS is committed to providing top-tier care for our student-athletes. As part of our Sports Medicine program, we offer ImPACT baseline testing, a computerized assessment that establishes a neurocognitive baseline. This test serves as an important tool in evaluating and managing concussions.

Administered on the Hilltop by our Sports Medicine Team, ImPACT testing is conducted at the start of each season for all Upper School athletes. Since younger students’ brains develop rapidly, baseline testing is not recommended for 5th-7th graders, as results quickly become invalid. Baseline tests remain valid for two years.

  • Athletic Training Facilities

The Athletic Training Room is located on the lower level of the Athletic Center next to the squash courts. It is open mid-morning until the conclusion of the final home game or practice each day. St. Luke’s athletic trainers can most often be found before games in the Athletic Training Room and on the fields, courts, and rinks with the student-athletes and their coaches.
  • Hospital for Special Surgery Partnership

SLS partners with the Hospital for Special Surgery (HSS), with Dr. Moira McCarthy serving as our team physician.

Dr. McCarthy is an orthopedic surgeon at HSS, specializing in arthroscopic knee and shoulder surgery. Learn more about Dr. McCarthy here.

For faster access to orthopedic care and injury evaluation, HSS offers multiple "Orthopedics Now" locations, allowing patients to book appointments quickly through the Ortho Now website.
